Investigation Summary
At approximately 7:15 a.m. on June 16, 2004, Employee #1, a sand re-claimer operator, was working alone operating the machine. He noticed sand was not coming out of the machine so he stood on a trash can and hit the sand hopper with a rawhide mallet several times. When this did not cause sand to come out, he stuck his hand up the unguarded outlet of the air lock feeder valve. Unknown to Employee #1, there were revolving vanes or blades inside the air lock feeder valve. These revolving parts trapped his right thumb between the revolving parts and the wall of the valve. Employee #1 was hospitalized with an amputated thumb.
