Backing Fork Lift Crushes Employee's Foot

At approximately 6:00 p.m. on May 14, 2004, a coworker placed a pallet in the on the shrink wrap machine. Employee #1 placed the shrink wrap around the boxes. Once the shrink wrapping was complete the coworker raised the pallet approximately 7 in. above the floor. Employee #1 was standing in front of the fork lift. It was then that the coworker noticed a black mark on the face of Employee #1. The coworker told Employee #1 about the black mark and Employee #1 rubbed the area and asked if the mark was still there. The coworker said yes and this question and answer were repeated several times. The coworker told Employee #1 to look in the rest room mirror or in the window of the marketing office door. Instead Employee #1 tried to look in the right side mirror of the sit-down fork lift. The mirror could not be adjusted so Employee #1 was unable to see the mark. Employee #1 then moved to the rear of the fork lift thinking she could see the mark from that position. The coworker said she looked to her left and saw another coworker tapping his fingers and waiting to unload a trailer at dock Number 14. The coworker told Employee #1 to move because the other employee wanted to unload the trailer. The coworker said she looked over her left shoulder and did not see Employee #1 so she thought Employee #1 had moved out of the area. The coworker began backing up the forklift. Employee #1 had moved a little to her right but was still standing behind the forklift when the coworker backed over her foot. As soon as the coworker heard Employee #1 yell the coworker drove the forklift forward. Employee #1 suffered a crushed right foot and three fractured toes and was hospitalized. .
