Investigation Summary
On February 10, 2004, Employee #1, a maintenance mechanic, heard a grinding noise from one of the conveyors located in one of the lines in the flat goods department of a warehouse distribution center. Employee #1 proceeded to determine where the noise was coming from. He used an 8-ft frame ladder to climb upon the beam that was approximately 9 ft and 3 in. Employee #1 walked on the beam, first to the blue colored beam, then to the gray colored support beam at 9 ft 10 in., a few inches higher than the blue support beam. The support beams were narrow, approximately less than 8-in. wide. Employee #1 was not wearing fall protection or fall restraint, while inspecting and listening to the noise. He checked 2 motors and lost his balance, while investigating the third motor. In an attempt to prevent his fall, he instinctively grabbed on the roller of the conveyor. His hand was caught between the end roller and snubbed roller. Fortunately, the conveyor stopped. It stopped automatically when the line was full. Employee #1 was able to pull his hand, walked across the beam, then climbed down the ladder and radioed for help. Employee #1's left hand was lacerated. Employee #1 did not have access to safe working platforms for elevated work locations.
