Investigation Summary
Around 8.30 a.m. on March 17, 2004, Employee #1 was working for Remedy Inc. at a warehouse facility controlled and operated by Delta Products Inc. The employee was moving a load of computer boxes, two stacks, one on top of the other, with a manual pallet jack, Serial Number 247250, from inside the warehouse to the loading area in the backyard when the top stack of boxes became unstable and fell on him. Employee #1 had been working there for about five months before accident occurred. The load on pallet jack was comprised of two stacks of 16 boxes placed one over each other and each box weighed about 30 pounds. The pallet jack was inspected during the walk around with a management official and found to be functioning alright on inspection day. He suffered a fracture to his left shoulder and contusion to back and ribs when a load of computer boxes fell from a hand driven pallet jack during material handling. He was transported to Queen of the Valley Hospital' in West Covina for medical treatment and was hospitalized in there for about 8 days due to a fractured shoulder.
