Investigation Summary
At around 11.00 a.m. on December 11, 2003, Employee #1, of AM Electric Inc., was a licensed electrical sub-contractor for about 30 years and had 12 employees on his payroll. A crew of four employees including the foreman was sent to Golden State Foods facility for some electrical repair work. Employee #1 was installing a circuit breaker on a cabinet type panel, 480-volts, when the accident occurred. He turned the power off on panel initially, went to see an electrical exit door located about 50 feet away which was not opening fully for the forklifts to go through, returned to panel and turned the power on. He went again to see if the same door was opening fully or not, came back to panel again and started working on open exposed part of panel for installing a circuit breaker and forgot to turn off the power this time due to distraction. During installation of circuit breaker his hand wrench shorted between the bus bars resulting in an electric flash that engulfed both hands inducing second-degree burns. Employee #1 was transported to Queen of the Valley Hospital for medical treatment and surgery.
