Investigation Summary
On September 4, 2003, as an inspector was doing a separate accident investigation, another accident which looked like a serious injury was noted that had been reported in the OSHA 200 log. On December 6, 2001, Employee #1 received a third degree burn on his hand while welding. Employee #1 was no longer with the company, but the employer was able to provide the inspector with a forwarding number. Employee #1 came in and showed the inspector his hand. There was no permanent disfigurement.
