Investigation Summary
At approximately 7:15 a.m. on August 1, 2003, Employee #1 fell from the wall of a security gate. Employee #1 arrived at the facility at approximately 7.00 a.m. and tried to open the security gate with his card key but was not successful. Employee #1's card key did not function at that time and he decided to enter the facility by climbing over a 12-ft high wall. Employee #1 fell in the act of crossing over to other side. No one witnessed the accident until a few minutes later. Employee #1 did not use any ladder nor did he contact anyone for assistance. Employee #1 fractured his right ankle and sprained his leg. Employee #1 was hospitalized.
