Investigation Summary
At 10.35 p.m. on June 12, 2003, Employee #1, a temporary employee of Ropak was working in the production department. Employee #1 was sent by her employer, Ultimate Staffing Services, to work for the Ropak Packaging Company in La Mirada and had been working there for about 4 months before the accident happened. Ropak Packaging has been in the business of manufacturing plastic buckets at the above facility for the last 25 years and has a combined total of 140 temporary and regular employees working there. According to the controlling employer, Employee #1 pulled excess half-inch wide plastic strap from a coil while packing a bundle of plastic buckets in the production area and fell when her left foot got entangled with the same strap. She fell to the floor and fractured her left femur. Employee #1 declined to be interviewed about her accident and there was no direct witness to the accident. Employee #1 was transported to the La Palma Intercommunity Hospital for treatment and was hospitalized for three nights for a fractured femur.
