Employee Is Injured in Fall from Fork Lift

At approximately 5:45 p.m. on August 7, 2002, Employee #1 was standing on the forks of a forklift that were elevated, in order to retrieve some boxes that were wedged at the back of a storage rack. As he moved to the front of the pallet to reach the jammed boxes, the pallet tipped over and he fell 12 ft, landing on a concrete floor. Employee #1 was transported to St. Francis Hospital in Lynwood, CA, where he was hospitalized and treated for cracked ribs and a fractured and dislocated left hip. He was released twenty days later. During the accident investigation, it was revealed that the pallet on which Employee #1 was standing was not secured to the forks or to the mast of the forklift. Additionally, there were no guardrails in place. The supervisor was not present at the time of the accident, but apparently was aware of the use of fork-mounted pallets as elevated work platforms. The employer was cited for a violation of T8CCR 3657(a).
