Investigation Summary
On June 15, 2002, Employee #1 and several coworkers were working at Sonora Bakery, a division of Crown Bakery, located at 4484 E. Whittier Boulevard in Los Angeles, California. The employees were removing dough from the mixing bowl of a 240-volt, 6.5-horsepower Treemime Mixer, Impastatrice type (Model Autfissa IS 160, Serial Number 96S137). The employees would stop the mixer by raising the guard covering the mixing bowl and the spiral to reach in to remove dough. They used the guard interlock instead of the stop button. The mixer had a jog button that allowed the bowl to turn with the guard open but not the spiral (mixing blade). Employee # 1 reached into the bowl to remove some dough and the guard fell down, which caused the mixer bowl to turn and the spiral to also turn. Employee #1 was standing on the right side of the mixer and was about to retrieve the dough near the back of the bowl when the mixer spiral became activated striking his right wrist. When Employee #1 yelled, a coworker turned and saw the guard was open and the spiral was turning. Other coworkers stated that the guard was open but the spiral was not turning. Employee #1 stated that his coworkers had removed dough the same way he had attempted to at the time of the accident and that the mixer had been malfunctioning. He stated that the machine, at times, would not start after he closed the guard even after pressing the start button several times and that, sometimes, the machine would start with the guard open. Coworkers said they had seen the mixer run with the guard up two weeks to a month before the accident. The employer said he did replace the micro-switch for the guard a few weeks before the accident. Employee #1 was transported to USC Medical Center where he was treated for 4 days for a fractured right wrist, lacerated right hand and damaged tendons.
