Investigation Summary
At approximately 11:00 a.m. on May 9, 2002, a forklift in the street hit a delivery truck driver. The forklift driver was driving the forklift with two boxes in front of him, impairing his vision. Each box was 39-inches high. The forklift driver hit the delivery truck driver who was standing behind his truck. The employee was transported to Suburban Medical where he received treatment to both his legs. One of his legs was crushed in the accident as it was pinned by the forklift against the back bumper of the delivery truck.
