Employee's finger amputated in machine

At 6:30 a.m. on April 22, 2002, Employee #1, a set-up mechanic with Carl W. Newel, was adjusting the setup and freeing a jammed knockout rod on a #31 Kawaguichi Injection Molding Machine (Model Number JEKS 265, Serial Number S8-E73548). His left index finger became caught between a moving washer and the face of the molding machine. Employee #1 was hospitalized for amputation of his left index finger.
