Investigation Summary
On April 8, 2002, Employee #1 was using a 120 Volt, Makita-Grinder (Serial Number 2779E) to grind a metal kitchen top. There was a flash, caused by an internal wiring short in the portable grinder. The flash struck the employee's right hand. The employee suffered an electric shock and fell to the floor. A coworker, who was in the same area, ran to help the employee, after seeing him fall to the floor. The coworker unplugged the grinder, and notified the management officials for help. The paramedics were called, and the employee was taken to the hospital. Employee #1 was hospitalized. The investigation revealed that the grinding tool's power supply cord was badly frayed and bare conductors were visible on the cord. There was a 3 in. long sleeve covering the bare conductors but flash traveled thru the sleeve and hit employee's right hand.
