Investigation Summary
At approximately 1:00 p.m. on July 8, 1998, Employee #1, a supervisor for USA Steel Erectors, Inc., was working on a steep sloped roof with two other employees. The project consisted of two aluminum skinned buildings located in Pedigo Park, which is located in Livingston, Texas. The work involved raising aluminum panels with ropes onto the roof. Each panel measured 32 feet 3 inches by 2 feet. The roof was complete except for approximately two more panels. The panels were being raised onto the incomplete side of the roof. To raise the panels, Employee #1 was positioned on one side of the panel, the second employee was positioned on the other end of the panel, and the third employee was standing behind Employee #1. The employees positioned on the ends of the panels grabbed the panels and walked them backwards over the I-beams and perlins to where the panel was installed. One employee was on top of the roof and another employee was on the side of the building on a ladder. Together, they would then attach the panel to the roof. Employee #1, upon returning from lunch, decided to work on the roof. While the previous crew was walking the panels backward, Employee #1 lifted the panel over his head and pushed it back to the installers. The employee on the other end continued to walk his end back. Employee #1 had one foot on top of a pitched roof I-beam and the other foot just inside the building on the top of a vertical I-beam. Employee #1 slipped and fell to the ground. The distance of the fall was approximately 25 feet 2 inches. Witnesses believe there were three factors that may have caused him to slip and fall. They were: a gust of wind, a lost grip in conjunction with lifting the panel over his head, and the inability to shift and turn his feet on the inside of the building. Employee #1 was taken to Herman Hospital where he died on July 11, 1998, at approximately 5:00 p.m. from massive blunt trauma to the head.
