Investigation Summary
Employee #1 and two coworkers were on an approximately 14 to 15 ft high aerial lift. They were hanging a door when it sprung open and struck either one of the employees or the aerial lift. The lift tipped backward and fell, with the workers, onto the concrete floor. Employee #1 suffered a crushed skull and was killed; one of his coworkers suffered lacerations to his thumbs and fingers.
