Employee injured when fingers amputated

At approximately 9:15 p.m. on November 9, 1998, Employee #1 was performing sanitation clean up work when he slipped. He fell against the guard on the top of an airlock star valve dislodging it. Employee #1's hand entered the rotary airlock blade housing. The rotating blades amputated four fingers of his left hand. Employee #1 was hospitalized.
