Investigation Summary
At approximately 12:20 p.m. on January 31, 2005, Employee #1, a shop helper working for a cabinet shop that manufactured kitchen cabinets for residential remodeling, was cleaning up the area under a table saw. He placed his hand on the saw table and, as he outstretched his hand, it contacted the blade of the saw. Several fingers were amputated and he was hospitalized.
