CONSTRUCTION EMPLOYEE KILLED IN FALL THROUGH FLOOR OPENING

EMPLOYEE #1 WAS INVOLVED IN THE DEMOLITION AND DISMANTLING OF A BOILER. HE WAS ASSIGNED AS A RIGGER. AFTER HE HAD REMOVED A VENT FROM THE RELIEF VALVE OF BOILER #3 HE REMOVED THE SAFETY BELT HE HAD BEEN WEARING SO THAT HE COULD GO ON BREAK. HE APPARENTLY TRIPPED OR LOST HIS BALANCE AND FELL THROUGH A 50 BY 29 INCH OPENING IN THE GRATING USED TO LOWER MATERIALS. HE FELL 35 FEET, 9 INCHES TO THE GROUND FLOOR. A TEMPORARY GUARD RAIL OF NYLON STRAPPING WAS AVAILABLE, BUT NOT IN USE, AT THE TIME OF THE ACCIDENT. EMPLOYEE #1 WAS PRONOUNCED DEAD AT 9:30 AM AS A RESULT OF HEAD INJURIES SUSTAINED IN THE FALL.
