Employee's Finger Is Amputated in Chain and Sprocket

At approximately 2:00 p.m. on February 16, 2004, Employee #1 was working at the Blackhawk Furniture Company as a helper stacking and placing wood on the waste conveyor. A piece of wood got stuck in the waste conveyor's chain and sprocket. The employee tried to remove the piece of wood with his right hand while the chain and sprocket were still moving. The chain and sprocket were not guarded and Employee #1's right index finger was caught and amputated. Employee #1 was hospitalized.
