Investigation Summary
On November 21, 2003, Employee #1 worked for a marine carpentry shop, and was making an angle cut of a plastic part on an unguarded table saw. The plastic part kicked back causing his right hand to roll downward toward the rotating blade. Employee #1's right middle finger came into contact with the blade. Employee #1 sustained severe lacerations, bone fracture, and bone loss. Employee #1's finger was surgically amputated.
