Employee amputates two fingers using an unguarded table saw

At approximately 6:00 a.m. on October 29, 2003, an employee was working at Illusion Furniture, a furniture manufacturer that glued wood and cut cabinet doors. The employee was using a Shopsmith table saw (Model Number 10 ER, Serial Number 72741) in the yard, cutting a piece of wood. When he heard a sudden noise, he looked away from the running saw. His left hand ran over the top of the saw. The saw did not have a blade guard in place. The employee sustained an amputation to his left thumb, left index finger, and lacerated his middle left finger. The employee was not hospitalized.
