Investigation Summary
At 1:30 p.m. on July 20, 2003, Employee #1, a bakery worker, was attempting to clean a series of packaging machines. As he began work on the third machine, which consists of several large conveyor belts for transporting the finished product, he noticed that there was an area under the conveyor that needed additional attention. Without deenergizing the machine, he used his left hand to wipe the conveyor with a rag. The rag got caught in the roller of the conveyor and pulled his arm in. An onsite mechanic heard Employee #1 screaming and immediately put the machine into the OFF position, releasing Employee #1's arm. Employee #1 was hospitalized and treated for a crushed left wrist and forearm.
