Investigation Summary
On May 14, 2003, Employee #1, working for the Pure Flo Water Company, which manufactures bottled water, was attempting to help repair a chain conveyor. This repair task was not his normal job. Employee #1 got his right middle finger caught between the chain and end sprocket. The chain and sprocket were not guarded. Employee #1 amputated the distal phalanx of his right middle finger and was hospitalized.
