Investigation Summary

Investigation Nr: 201690211
Event: 04/28/2001
Employee injured while performing maintenance on a crane

Employee #1 was removing a damaged load drum from Crane #8, which weighed 3,500 pounds. He knocked out the final inside bolt from the gear side's pedestal bearing using a brass drift pin, held in his left hand with a hammer. The drum's gear side was secured by rigging but it still moved forward. Employee #1's left thumb and a portion of left index finger were caught and crushed between the pedestal bearing and another portion of this drum assembly. His left thumb was amputated at the joint just below the nail, and a portion of the skin was removed from the top section of his left index finger. He was hospitalized.
