Investigation Summary
At approximately 11:00 a.m. on December 23, 1996, Employee #1 was on a roof and was loading staples into his hammer tacker. He dropped the hammer tacker and he began to chase it when he slipped and fell approximately 20 ft to the ground. Employee #1 suffered back injuries that required hospitalization.
