Investigation Summary
On December 4, 1997, Employee #1 and three other employees were laying the roof decking for a new gymnasium at Puget Sound Naval Shipyard. Three of the employees were using personal fall arrest equipment connected to horizontal life lines, while Employee #1 was not using any fall protection. Employee #1 walked to an exposed edge carrying a welding lead, apparently to tack down the newly laid decking. Employee #1 probably tripped and then fell, approximately 38 ft to the floor. He sustained blunt-force injury to the head and was transported to the hospital. Employee #1died two days later.
