EMPLOYEE KILLED WHEN HE FELL 18 FEET

On January 20, 1998, Employee #1 was climbing from a ladder to a work platform. Employee #1 was climbing the ladder rungs of an integral prefabricated scaffold frame while carrying a grinder. When Employee #1 reached the top planking of the scaffold he placed the grinder onto the platform. Employee #1 then reached to grab the guardrail post to pull himself onto the platform but missed the post. This caused Employee #1 to lose his balance and fall backward 18 feet landing on the sidewalk. Employee #1 struck his head on the sidewalk. Employee #1 died the following day, January 21, 1998. The cause of death was multiple skull fractures with associated cerebral cortical contusions and subdural hemorrhage due to blunt trauma to the head.
