Investigation Summary
Employee #1, a roofer, was using a submersible pump to remove water from a 19 ft 9 in. roof. He was working alone, standing on an unsecured aluminum extension ladder. Employee #1 fell from the roof or ladder and sustained extensive injuries to his head and to the left side of body. He was killed. The coroner found no sign of electric shock.
