Investigation Summary
Employee #1 was operating a food packaging machine when she removed a broken guard that covered a chain and sprocket. The tip of her right index finger became caught by the pinch point of the chain and sprocket, amputating approximately 1/8 in. from the tip. She was transported to the hospital, where she was treated and released. The pinch point was not guarded.
