Investigation Summary
Employee #1 attempted to climb into a waste skip, which was elevated approximately 30 feet by a fork lift. The skip tipped, dropping Employee #1 out onto the concrete surface below. Employee #1 was not protected by any fall protection. Eagle Environment does provide fall protection training for their employees on a monthly basis. Fall protection equipment was on site at the time of the accident but was not being utilized by the Eagle Environmental employees. Employee #1 was attempting to remove bagged asbestos waste. Instructions were given to the employees not to cover the lift cables, which were attached to the waste skip. At least a portion of one cable was buried. Employee #1 entered the waste skip to uncover the cable. He pulled on the cable causing the skip to tip him out onto the concrete floor 30 feet below. Employee #1 was reported to have entered the elevated skip the day before the accident and he was not reprimanded by Eagle Environmental. The accident occurred when Employee #1 entered the waste skip a second time. Note that the fork lift, its operator and waste skip were all controlled by Jacobsen Construction, who was trying to aid the asbestos abatement crew.
