Investigation Nr: 201510229
Event: 03/23/2004
Employee injured in front end loader accident

At approximately 7:15 a.m. on March 23, 2004, an employee on a Dresser 560 front end loader was travelling down a 15 grade approximately 120 ft long to reach the assigned loading area. The left front brake caliper on the front end loader failed during the grade descent, resulting in the front end loader striking the rear of a parked gravel haul truck. The proper brake action was verified by the front end loader operator and the HDR mechanic prior to descending the grade. The employee suffered a shoulder strain and rib contusions but was not hospitalized.
