Investigation Summary
At approximately 12:30 p.m. on February 27, 2003, Employee #1, a supervisor, was selecting orders for shipment in the combine area of a milk plant in San Leandro, CA. In the combine area, cases selected from three lines (A, B, and C) were combined into one line. Once the cases were combined, they were directed to three stackers where they were stacked and then combined back into one lane again. At the stacker, the cases entered the stacker one at a time. A lifter lifted the first case up while the second case entered the stacker. The lifter then came down again to lift the second case. The process continued until the desired number of cases was stacked. Employee #1 noticed that a stack of five cases in Line C did not come down. He walked over to the line and tried to manipulate the controls to bring the stack down. As he did this, the stack started to fall over. He tried to stop the stack with his left hand but the cases kept falling and two of his left fingers were caught between one of the cases and a metal railing. His middle finger was amputated at the first joint and his ring finger was lacerated and broken. He was hospitalized with his injuries.
