Investigation Summary
At 1:30 p.m. on February 22, 2003, Employee #1 was assisting two workers working at the document input conveyor to sort out non document debris. Employee #1 reached into the input chute to remove a bulky bound binder to prevent a jam of the shredder. The shredder's crusher (sweep-arm) paddle caught Employee #1's right hand, amputating his index and middle fingers. He was hospitalized.
