Investigation Summary

At approximately 11:50 a.m. on October 15, 2002, Employee #1 was standing at the back end of his company-assigned, 16-foot, Model Number 4,400 International bobtail truck. It was part of his assigned duties to use a pallet jack to move pallets of scrap paper around in the truck bed to prepare them for off-loading by a fork lift operator. He stated that he was manipulating a pallet into position to be removed. He was near the rear end of the truck bed. He lost his balance and stepped back (further toward the rear of the truck) with his right foot. His foot went between the end of the truck and the top of the lift gate. The lift gate was in the lowered (stored) position. He then lost his balance and fell off the rear of the truck. However, his foot was still caught between the truck and the lift gate, fracturing his leg. He fractured his leg as a result of stepping into the space between the rear end of the truck bed and the lift gate of the truck, then falling off the truck as a result of the imbalance. He was hospitalized.
