Investigation Summary
At approximately 2:50 p.m. on July 29, 2002, Employee #1, a truck driver, was using a lift truck to off-load a dozer blade from a flatbed truck. The blade fell off the forklift and struck Employee #1, who sustained a fractured rib and internal injuries. Employee #1 was transported to the hospital where he was hospitalized for approximately seven days and underwent surgery for internal bleeding.
