Investigation Summary
On May 7, 2007, Employee #1, the designated competent person, was rigging beams and directing crane operators. There were two cranes being used to lift the beams. The beams weighed 300 pounds per foot and were of varying lengths. He placed himself in an area where loads were to be lifted. He rigged one end of a beam instead of rerigging it; he took a shortcut by using 4-in. by 4-in. board. Employee #1 was struck in the head by the beam, causing a concussion and was hospitalized.
