Investigation Summary
On August 21, 2006, Employee #1, a solar panel installer, was on an extension ladder, adjusting pipe to solar panels at the edge of a residential roof. When the ladder slipped, he fell approximately 10 ft. Employee #1 was hospitalized for a concussion and a fractured wrist.
