Investigation Summary
At approximately 2:30 p.m. on September 29, 2004, Employee #1 was installing a new roof on the first floor when he slipped and fell. He fell approximately 11 ft off the roof onto a concrete driveway. The roof was not abnormally steep nor was there debris on the roof. Employee #1 suffered bruises to his arm, shoulder, and head, but sustained no broken bones. He was hospitalized for injuries sustained in the fall.
