Employee killed in fall from scaffold

Employee #1 apparently climbed the end of the scaffold to see the work that was being done. It had started to rain shortly before the accident, and it is likely that his shoes were wet, which may have caused his foot to slip. He was killed when he fell from the outside of the scaffold, struck a guardrail, and then fell to the next lower adjacent concrete floor level.
