Investigation Summary
On May 8, 1998, Employee #1 was building a culvert over a creek. Employee #1 was using a Case 580K Construction King to move topsoil mixed with straw to build the road. Employee #1 was backing up near the edge of the culvert, when the soil underneath the rear left tire collapsed. As the tractor began to turn over, Employee #1 jumped off the tractor. Employee #1's hand was pinned under the arm of the shovel. His chest was pinned under the front left ROP support. His head was pinned under the engine compartment. He was killed. The tractor was equipped with a certified ROP, but was not equipped with a seatbelt.
