Investigation Summary
Employee #1 drove a Ford Escort onto a front end alignment rack with the assistance of another employee. Employee #1 placed one hand on the bumper of the car and was in the process of connecting an air hose to the air jack when the car rolled forward off the rack. Employee #1 fell backwards striking his head on the rear wall of the pit, and was pinned beneath the car. Employee #1 was killed.
