Investigation Summary
At approximately 10:00 a.m. on October 7, 1996, Employee #1 was working alone, using a forklift to stack bundles of boxes. Apparently, he had dismounted to rearrange the stack of boxes or to reposition the forks. It is surmised that the stack of boxes may have fallen, striking Employee #1 and forcing his face and head against the forks and/or the mast. He suffered broken bones above his right eye, a broken cheek bone, and a broken nose. Employee #1 died in the hospital on November 16, 1996, from respiratory arrest and complications arising from his initial injury.
