Investigation Summary

An employee was working on his first day on the job. The shift started at 4:30 PM the day before. He had been hired as a welder and fitter and, about 5 minutes before the end of the shift (at about 2:55 AM), he had swept his workstation and was gathering up his personal belongings. Witnesses heard a noise but did not see him fall. They found him on the concrete floor, lying on his back. The injured employee was transported to a hospital, where he died of his injuries at about 6:00 PM the following day. The medical examiner listed the cause of death as severe trauma to the left side of the head above the left ear.
