Investigation Summary

Employee #1 died when he was crushed by a fork lift. Two operators were instructed to move two fork lifts from a shed to a dock area, because the fork lifts were needed for a task. One of the operators drove his fork lift from the shed and was waiting for the other driver. The other operator was driving a 1985 Baker fork lift equipped with front wheel drive and rear wheel steering. The Model Number was B150. Employee #1 entered the shed and approached the Baker fork lift from behind the operator, while the operator was going forward and making a right hand turn. Without alerting the operator of his approach and without being seen by the operator, Employee #1 entered into the path of the fork lift while it was turning. Employee #1 was struck by the fork lift or caught by one of the protruding rear steering wheels. He died when he was crushed by the fork lift.
