EMPLOYEE KILLED IN FALL THROUGH WALL OPENING

At 11:45 a.m. on December 24, 1997, Employee #1 and a coworker were installing insulation on a second floor wall. The coworker was working from a 6-feet high ladder, and Employee #1 was working from the floor. Employee #1 was installing insulation onto a block wall when he either lost his balance or misstepped, and fell through the inadequately guarded wall opening. There were studs present in the wall, but he fell through them. He fell 13-feet-10-inches to the concrete floor. His head struck the concrete floor. CPR and emergency medical treatment were given to Employee #1, but he died from massive head injuries the next day due to the blunt impact to his head.
