EMPLOYEE KILLED IN FALL FROM LADDER

Employee #1 was going up a 20-foot long metal extension ladder to a height of 14 feet to a roof when the ladder either slid side ways, or the feet slipped out from under the ladder. Employee #1 fell, striking his head on the concrete surface and was killed.
