TWO EMPLOYEES INJURED IN FALL

At approximately 12:50 p.m. on December 4, 1997, Employee #1, Employee #2, and a coworker were being raised 9 ft by a forklift truck while on a platform scaffold. The platform was incorrectly attached lengthwise to the forklift forks, and the forks came through the plywood floor, allowing the far end to fall to the ground. Employee #1 and Employee #2 and were injured when they fell on top of each other. Both employees were hospitalized.
