Investigation Summary
At approximately 9:45 a.m. on February 9, 2004, Employee #1 and a coworker were moving an oxygen tank from the street to the location where he was working. According to the foreman, the tank weighs approximately 700 pounds to 800 pounds and was supposed to be moved using a forklift. Also available was a cork designed to transport compressed gas cylinders. Employee#1 chose the cork instead of the forklift since trucks parked on the narrow street made its use more difficult. After moving the tank approximately 200 ft, with Employee #1 pulling the cart and the coworker pushing it, they reached a debris ramp going down. Employee #1 instructed the coworker not to push the cart. Employee #1 pulled the cart down the ramp by himself. The cart wheel went into a depression (hole) and started to tip. Employee #1 tried to stop it from falling but he could not. The bottle and cart hit Employee #1's leg below the knee, knocking him down. Employee #1 suffered multiple fractures of his left leg and was hospitalized.
