Investigation Summary
At approximately 2:00 p.m. on On October 15, 2003, Employee #1 and a coworker were on the second floor constructing the interior wood trimming. They were using a Makita table saw, Model Number 2703 and Serial Number 120737A. This table saw was a manual-feed circular ripsaw with no installed guard, antikickback device, or splitter. A 10-in. carbide blade was installed. The employees were rip-cutting wood of 0.625-inch thickness. Just before the injury, another coworker had used the saw. When Employee #1 used the saw, he used it without a pushstick or pushblock, and without the use of the fence or a miter gauge. The employee freehand cut a non-square thin wedge, dimensions 0.25-in. maximum width, sloping to a point, 21-in long, from a starting piece of 37 in. long and 1.25 in. wide. The employee's left hand was placed on the length of the workpiece and his right hand was pushing the base of the workpiece toward the blade. His left hand slid into the unguarded blade when the wedge was at the point of breaking off from the starting piece. Employee #1's left-hand middle and index finger were amputated and his thumb and ring finger were lacerated, resulting in two days of hospitalization. Employee #1's doctors were unable to reattach either finger due to the shattered bone.
