Investigation Summary
At about 6 p.m. on June 3, 2003, Employee #1 conducted light maintenance on an Alimak 27/37C hydraulic construction personnel hoist (CPH), Manufacturer's Number 1975. He was working at a site in San Francisco, California where two high-rise condominium towers were being constructed. Employee #1 was finishing greasing the CPH counterweight from the roof of the cartop when he was interrupted by a coworker who needed a ride from an upper floor. Employee #1 returned to the car cage, picked up the coworker and delivered him to the bottom landing . When doing that, the CPH overshot the landing limits because Employee #1 inadvertently left a cartop removable operating lever in place. The CPH could be operated from the cage and the cartop, which became stuck in the cartop guardrail. Employee #1 climbed to the cartop and proceeded to operate the hoist from the cartop in an attempt to examine the problem with the landing limits. As the CPH was moving upward, Employee #1 looked over the cartop guardrail and his head was crushed between the guardrail and the mast tie-in to the building. Only a 4-in. horizontal distance existed between the metal guardrail and the tie-in. Employee #1 sustained a fracture and laceration to the head. He lost 6 teeth and was hospitalized.
